Beyond Therapy: How Skilled Nursing Supports Recovery

Recovery after a hospital stay may involve more than rebuilding strength and mobility. Some residents also require medication management, clinical monitoring, wound care, respiratory support, or assistance managing complex medical conditions.

At Heritage Manor Nursing & Rehabilitation, rehabilitation and skilled nursing work together to support each resident’s medical needs and recovery goals.

Rehabilitation and Clinical Care Working Together

Depending on the resident’s needs, rehabilitation may include physical, occupational, and speech therapy. Therapy can help residents work toward improved mobility, strength, communication, swallowing, and greater independence with everyday activities.

Between therapy sessions, skilled nurses provide ongoing clinical support. They monitor changes in condition, administer medications, coordinate with physicians, and help ensure that the resident’s care plan reflects their current needs.

Skilled nursing services may include:

  • Medication management
  • IV therapy
  • Wound care
  • Diabetes and pain management
  • Tracheostomy care
  • Support for complex medical conditions

Having therapy and skilled nursing available in one setting allows the care team to communicate regularly and provide a more coordinated recovery experience.

Preparing for the Next Stage

Families are an important part of the care process. Throughout the stay, the Heritage Manor team provides updates about progress, changing needs, and discharge planning.

As the resident prepares for the next stage of care, the team may provide caregiver education, recommendations, and coordination with physicians or follow-up providers.

At Heritage Manor Nursing & Rehabilitation, our goal is to support each resident as they work toward improved function, greater strength, and a more manageable transition home or to the next level of care.
